2024-25 Statement of Educational Purpose

2025 - 26 (Fall 2025 - Summer 2026)

Students can be selected for Identity Verification either by CSN or by the Department of Education. Please review your To Do list for the type of ID verification you must complete.

In-Person Option
If your MyCSN To-Do List requests verification of ID, please visit your nearest campus and bring your unexpired, government-issued ID.

Other Option(s) - Requires documents to be NOTARIZED (no online notaries are accepted)

If you are selected by CSN for ID review and are unable to come in person, you may submit the form below along with a copy of your unexpired government-issued ID either by mail or email.
If you are selected by the Department of Education for verification and cannot appear in person, your only option for ID verification is by mail (no email). The form can be submitted along with a copy of your unexpired government-issued ID and mailed to the address on the form.

Satisfactory Academic Progress Appeals and Reinstatements can require extensive effort and documentation. Before getting started, please review the video below or visit one of our offices to ensure you understand the process and requirements.

Things to consider

Semester Payment Deadlines are three weeks before the term begins. The Office of Financial Aid does not hold classes for students in the process of an appeal review. Please discuss payment arrangements with the Cashier's Office.
Students cannot submit a SAP Appeal or SAP Reinstatement if they are not suspended. Our Office evaluates Satisfactory Academic Progress 8-10 days after a term ends.

Fall 2025
Students may begin submitting SAP Appeal and Request for Reinstatements for Fall 2025 beginning July 4th, 2025 at 12:00 a.m.

We highly encourage students to consult with the Office of Financial Aid before submitting any appeal. Incomplete appeals are subject to denial. Please read all directions carefully and submit all documents in one complete submission.

Please ensure that you are enrolled in the semester(s) for which you are requesting a loan and that your enrollment is at least 6 credits leading toward your declared major for the term. Note: Students must have course enrollment that exceeds 30 days to be eligible for a loan disbursement.
